Reducing Older Adult Isolation
Goal
Supported by a grant from the Foundation, a large collaborative of organizations is working to end isolation among older adults with a multi-pronged approach: identifying at-risk older adults; connecting them with resources; and educating others about the health impact of social isolation.
Through its work, the group identified opportunities for some siloed support services to better collaborate. A subsequent three-year pilot, also funded by the Foundation, will allow the group to add more referral agencies and ultimately allow community self-referrals. Results and strategies also will be shared with organizations around the country to expand the impacts nationally.
